Raintree acquires Spike Technologies to bring AI voice agents to therapy billing

Why now? Revenue cycle work is among the most labour-intensive parts of healthcare. Physical therapy practices face an average claim denial rate of roughly 13%, and nearly three-quarters of those denials must be appealed.
What's the endgame? Raintree is building toward fully autonomous revenue cycle management (RCM). Because the AI voice agents are native to its EMR rather than a third-party layer, they act with full clinical, scheduling, and financial context, completing multi-step work rather than following a fixed menu of options.
Chief executive officer Nick Hedges framed the ambition bluntly. "We are not just optimizing operational efficiency for our clients, we are launching a seismic shift that will slash RCM expenses to an estimated 1%," he said.
What changes for customers? The capability is built into the EMR practices already use, so there is no separate system to buy or integrate. Raintree says customers can expect less manual phone work, fewer touches per claim, and faster cash.
What could go wrong? The market is crowded with tools that Spike co-founder and chief executive officer Povilas Gudzius describes as "really just rule-based automation or a chatbot." Delivering agents that "genuinely reason and execute complex work, end to end" at scale — and proving the claimed cost reductions — remains unproven across practices.
The signal: Industry analyses estimate AI and automation in the revenue cycle represent up to $360 billion in potential annual savings, and vendors are racing to embed agentic AI into core healthcare systems. Raintree's bet is that owning the EMR, not bolting AI onto it, is what makes autonomous billing viable. The combined team plans to detail availability and roadmap at TherapyCon in Austin, running August 19–21.
