ROX Medical was a medical device company that developed a novel, implantable device to treat uncontrolled hypertension. Founded in 2003 by Rodney Brenneman and Keegan Harper, and headquartered in San Clemente, California, the company emerged as a spinout from Stanford University. Initially, ROX Medical explored treatments for conditions like chronic obstructive pulmonary disease (COPD) and heart failure. During clinical trials for COPD, a significant and beneficial side effect was observed: a notable reduction in patients' blood pressure. This discovery prompted a strategic pivot, and the company shifted its primary focus to developing a treatment for drug-resistant hypertension.

The company's core product was the ROX Coupler, a small, stent-like implant made of nitinol. The device was designed to be placed via a minimally invasive, catheter-based procedure lasting about an hour under local anesthesia. It creates a permanent, fixed-size passage, or anastomosis, between the iliac artery and vein in the upper thigh. This allows a controlled amount of blood to be redirected from the high-pressure arterial system to the low-pressure venous system, which reduces peripheral vascular resistance and improves arterial compliance, resulting in an immediate and sustained drop in blood pressure. The procedure was designed to be reversible. The ROX Coupler received CE Marking in Europe for treating both hypertension and advanced COPD.

The company's business model revolved around the development and commercialization of this device-based therapy, targeting a significant market of patients whose hypertension is not controlled by medication. ROX Medical conducted multiple clinical trials, including the CONTROL-HTN studies, to demonstrate the safety and efficacy of the Coupler, with results published in journals like The Lancet and Hypertension. Over its lifetime, the company raised approximately $101 million in funding through several rounds, with its last major round being a $40 million Series E in 2016.
