The core of Cardiorentis's research and development was its lead product candidate, Ularitide. This drug is a chemically synthesized form of urodilatin, a human peptide produced in the kidneys that regulates fluid and sodium balance. Ularitide was developed as an intravenous treatment for acute decompensated heart failure (ADHF), a life-threatening condition where heart function suddenly worsens. The drug works by binding to natriuretic peptide receptors, which helps relax smooth muscle tissues, leading to vasodilation and increased blood flow. In December 2015, the U.S. Food and Drug Administration (FDA) granted Fast Track designation to Ularitide for ADHF, aiming to speed up its development and review process.

Cardiorentis operated on a business model centered on drug development and subsequent commercialization, contingent on successful clinical trials. The company's financial strategy involved securing significant private funding to advance its clinical programs. Over its active years, Cardiorentis raised a total of $119 million through two major funding rounds from investors including HealthCare Royalty Partners and Daniel Gutenberg. A significant milestone was the initiation of the TRUE-AHF Phase III clinical trial in August 2012, a large-scale study involving over 2,100 patients across numerous centers in the U.S., Europe, Canada, and Latin America. The company also entered into a collaboration with Roche in 2014 to incorporate advanced diagnostic tests into the trial. However, the results of the TRUE-AHF trial showed that Ularitide did not sufficiently improve patient conditions, leading to legal disputes with the clinical research organization, IQVIA, over the management of the trial. The company is now considered inactive.
